Athletic Survey Online

Dear Parent/Guardian,

This year’s LSHS Athletic survey is being delivered to you via the web and will not be mass mailed as in the past. Please inform other parents of this arrangement and encourage their participation. The point of this change is to cut down on mass mailing and allow you ongoing access to the form. You can even print multiple forms to individualize for multiple students, a particular coach, or differing programs. It is meant to make the process more efficient – let us know what you think. We are going to try it this year and see what the response is.

To foster an effort of continuous improvement, we are asking that you PRINT, FILL OUT AND RETURN THE PARENT ASSESSMENT SURVEY LOCATED IN THE LINKS SECTION OF THE ATHLETIC WEB-SITE:

The Links section is on the right side of this page. If for any reason you can not open or print the document, please stop by the high school office to pick one up or contact us at 630-493-8300 to have a copy mailed to you.

Our office is collecting data from coaches, students, and parents to assess the things we do well, and those that we can do better. We value your input and hope that you will take the time to respond and return this survey to us. We are asking that you return these by August 15. We have extended the previous announced deadline to allow for more input. The information learned from the various sources will assist us in developing goals and action plans for program improvement and points of emphasis for the next school year.

These surveys are anonymous in nature, so please no signature or return address. Your honesty and sincerity is appreciated. We are proud of this effort and serious about the feedback we receive. Here is your chance to tell us what is wrong and what is right about Lisle Athletics – we look forward to hearing from you.
