DAILY HEALTH CERTIFICATION (IN-PERSON ONLY)

DAILY HEALTH CERTIFICATION IS REQUIRED TO ENTER THE BUILDING

It is extremely important that our learning community works together to maintain a healthy school environment.  To accomplish this, each student who is attending in-person is required to submit the daily health certification prior to entering the building.  Please do not send your child if he or she is exhibiting any COVID-like symptoms. Refer to the Return to School document for procedures and protocols.

 

GRADES 9-12

  • Each parent/guardian will receive a daily health certification email from CrisisGo at 5am.
  • Parents/guardians will screen their child and complete the online health certification form at home on the days their student physically attends school.
  • Students will scan their student ID upon entering the building to confirm certification.
  • Students that are uncertified will be asked a series of health related questions and will require a temperature check before gaining entry to the building.
  • If a student feels ill or exhibits any symptoms, he/she should stay home.

GRADES PK-8

  • A daily health certification ticket book was provided to each family at materials pick-up on October 14th.  If you misplace your booklet, you may print copies of the form on our website.
  • Each day a child physically attends school, the parent will screen their child and complete and sign the health certification verification form.
    Students will bring the verification form for the day and submit it to the designated staff monitor prior to entering the school building.
  • Students that are uncertified will be asked a series of health related questions and will require a temperature check before gaining entry to the building.  Parents will be contacted for younger students.
  • If a student feels ill or exhibits any symptoms, he/she should stay home.

K-8 DAILY HEALTH CERTIFICATION PACKETS

If you did not retrieve your packet on October 14th, please call the school office to arrange a pick up.

FOOD SERVICE INFORMATION

FREE SCHOOL LUNCH PROGRAM EXTENDED

The U.S. Department of Agriculture announced that it is extending the distribution of free meals to students through the end of the 2020-2021 school year. This allows Lisle 202 and Aramark Food Service to continue serving free meals to all children 18 years and under within our district to help ensure children have access to nutritious food as the COVID-19 pandemic continues.


Aramark will offer curbside pick-up of grab and go lunch at no cost for all children under the age of 18 on Mondays and Thursdays from 1:30p – 3:30p at Lisle High School on days school is in session. No pre-ordering is required.

 

RETURN TO IN-PERSON LUNCH SERVICE

All students will be offered free breakfast and lunch items.  School lunch menus are posted on the Lunch Menu page of our website (www.lisle202.org>Quicklinks>Lunch Menu).

  • Grades K-1: Students will be given "to-go" breakfast and lunch items to take home.
  • Grades 2-5: Students in physical attendance will be offered a hot or cold lunch option.  They will also be given the option to take "to-go" breakfast and lunch items.
  • Grades 6-8: Students in physical attendance will be offered a hot or cold lunch option.  They will also be given the option to take "to-go" breakfast and lunch items.
  • Grades 9-12: Students will be given the option to take home "to-go" breakfast and lunch items.

BUS TRANSPORTATION (IN-PERSON LEARNING)

2020-2021 Elementary and high school bus routes are now posted on the Transportation Page of our website.  Junior High routes are being updated and will be posted by Wednesday, October 21st.

 

BECAUSE IT IS WORTH REPEATING...

Reminder! All required medical forms must be submitted in order for students to attend in-person.  Forms, immunization requirements, and exam schedule requirements are available on the Health Information page of our website.

 

If your child has an asthma diagnosis, please submit the annual Asthma Action Plan from your healthcare provider to the school nurse.

COVID-19 Testing Facility Holiday Schedule

The DuPage County Health Department's Public Health Centers in Addison, Lombard, West Chicago, Westmont and Wheaton will be closed on Monday, Oct. 12, 2020, in observance of Columbus Day.

  • The DuPage County COVID-19 Testing Site will be closed on Monday, Oct. 12, 2020, in observance of Columbus Day.
  • The Health Department's Crisis Hotline (630) 627-1700 will remain in operation.
  • All offices will reopen at their regular times on Tuesday, Oct. 13, 2020.
  • The DuPage County COVID-19 Testing Site will reopen at 7:00 a.m., on Tuesday, Oct. 13, 2020. 

DuPage County COVID-19 Testing Facility

The DuPage County Health Department has created a short step-by-step video explaining what to expect when going through the testing procedures at the new DuPage County Drive-Thru Test Site. This video will describe the efficient process from beginning to end as well as provide helpful information to visitors so that they know how to prepare before their visit. Some of the information covered includes, how to access the test site, the information visitors will be asked to provide and the steps on how to perform the self-administered nasal swab test.
