Medical forms may be submitted in the following ways:
REMINDER: Please be sure to complete the Health History section of the physical form AND sign and date the parent signature box. The physical is not valid if this section is not completed.
- Drop off forms at the District Office
- If the forms are a print-out or scanned, upload into PowerSchool or email the forms to registration@lisle202.org
- Fax the forms to the District Office at 630.908.3243
- Mail the forms to the District Office at 925 Burlington Ave Lisle, IL 60532
Please note that we reserve the right to request paper copies of the medical forms.
The following Medical Forms are required for student attendance, though not all items are due at the time of enrollment. For detailed information, due dates, and information on low-cost resources, please visit the Health Services page of our website.
The following items are required:
- Physical Examination (Grades K, 6, 9)- Due by THE FIRST DAY OF SCHOOL, or before attendance can begin if enrolling during the school year. In addition, the physical must be dated within one calendar year of the first day of school.
- Proof of Immunizations (included on physical exam form) or Religious Objection to Immunizations Waiver
- Please visit the Health Services page of our website for details on required vaccines
- Proof of Immunizations (included on physical exam form) or Religious Objection to Immunizations Waiver
- Dental Examination Form or Dental Exam Waiver (Grades 2, 6, 9) - Due to the School Nurse by May 1st
- Eye Examination Form or Eye Exam Waiver - Due to the School Nurse by October 1st (Required for Kindergarten in a public, private, or parochial school and any student enrolling for the first time in a public, private, or parochial school in the State of Illinois shall have an eye examination.)
- Medication Authorization Form
- If it is necessary for your child to take either prescription or over-the-counter medication at school, a medication authorization form must be completed by a licensed health care provider. This form must be updated annually. A parent/guardian must also sign the form and bring the medication to school in a clearly marked pharmaceutical container.
