Step 1 - Complete Online Pre-Registration

Complete this information and submit the form.
Please be sure to select the appropriate year of enrollment.

Once you submit the Enrollment Pre-Registration Form, the school will process your registration. Upon approval, you will receive a confirmation email:

“Welcome to Lisle” email – sender: PowerSchool

Step 2 - Set-up PowerSchool Account & Complete Online Registration Forms

Use the “Welcome to Lisle” email you received from “PowerSchool” to set up your account and complete online registration.

Documents Required for All Students

We reserve the right to require the document be shown in person at the school office. (Parents can contact the appropriate county office vital records department to obtain the official birth record for their child. DuPage County 630-682-7400; Cook County 866-252-8974; Kane County 630-232-5950. Parents can also go to local Currency Exchange to obtain a birth certificate.)

	X	Junior High Students* – Unofficial transcripts, withdrawal grades and test scores from previous school if available High School Students* - Unofficial transcripts, withdrawal grades, and schedule obtained from previous high school For high school 9th graders entering mid-year and all 10th through 12th graders. These additional documents are needed in order to set an appropriate class schedule and ensure proper credit.

*Schools in the Lisle District 202 do not refuse to enroll a student because of a student’s failure to present his/her student permanent or temporary record from a school attended previously.

Step 3 - Registration Appointment

PK-5 - The school office will contact you via email once you have completed the online registration process in PowerSchool.
6-8 - The school office will contact you to set up a Registration Appointment once you have completed the online registration process in PowerSchool.
9-12 - The school office will contact you to set up a Registration Appointment.

Residency Documentation Information

RESIDENCY DOCUMENTATION INFORMATION

Parent/guardian, address, and date must be clearly shown on all residency documentation.
The parent/guardian listed on the residency documentation must match the photo ID.
Residency documentation must be within 60 days of enrollment/registration.
Upload all residency documents into Form C in the PowerSchool Registration Portal.
Scanned documents or photos of photo ID and residency documentation are accepted.
ONLY SCANNED COPIES of the school physical are accepted.

CATEGORY II - Residence Confirmation (One document required – choose for homeowner or renter) If you are not occupying a residence within the boundaries of the Lisle School District when you enroll your child, Board of Education Policy requires payment of one semester of tuition until occupancy occurs. The unused portion of the tuition will be refunded upon presentation of proof of occupancy. Please call the District Office at 630-493-8000 for further information.

Deed, Closing Settlement Statement, or most recent Mortgage Statement Real Estate Tax Bill – most recent which identifies the address and homeowner’s name – Form 1098, tax statement from DuPage County Property Lookup website: http://www.dupageco.org/PropertyInfo/PropertyLookUp.aspx Property Sales Agreement that includes the address and homeowner’s name.
Renter	X	Signed and Dated Lease – Dates must be within the current school year. If you do not have a lease document, you must complete and submit a Lease Affidavit Form.
Third Party Resident	X	Both Items Required Most recent real estate tax bill or lease agreement showing the owner/primary renter’s name Signed and Notarized Affidavit of Residency (Third Party Resident – the person with whom the parent/guardian and student lives with)
CATEGORY III - Address Verification (One document required showing proper address from the last 60 days) If you have just recently established residency in the Lisle School District, you may not yet have a document from this category. You will have 30 days from the time you enroll your child to produce the required documentation.

Step 4 - Submit Medical Forms to School Office

Medical forms may be submitted in the following ways:

Drop off forms at the school office
If the forms are a print-out or scanned, upload in PowerSchool or email the forms to registration@lisle202.org
Fax the forms to the school office
Mail the forms to the school office

Please note that we reserve the right to request paper copies of physical forms.

Printable requirements

The following Medical Forms are required for student attendance, though not all items are due at the time of enrollment. For detailed information, exclusion dates, and information on low-cost resources, please visit the Health Services page of our website.

Physical Examination - Due to the School Nurse by THE FIRST DAY OF SCHOOL
- Out of state transfers have up to 30 days to submit physical examination
Proof of Immunizations (included on physical exam form) or Religious Objection to Immunizations Waiver
- Please visit the Health Services page of our website for details on required vaccines
- 12th Grade students must provide documentation of the required meningococcal (MC4) vaccine
Dental Examination Form or Dental Exam Waiver - Due to the School Nurse by May 1st
- Kindergarten, Second Grade, Sixth Grade
Eye Examination Form or Eye Exam Waiver - Due to the School Nurse by October 1st
- Kindergarten and all new students
Medication Authorization Form
- If it is necessary for your child to take either prescription or over-the-counter medication at school, a medication authorization form must be completed by a licensed health care provider. This form must be updated annually. A parent/guardian must also sign the form and bring the medication to school in a clearly marked pharmaceutical container.

Fee Payment

The Board of Education waived the school book/materials fees for the 2024-2025 school year. Class fees and other athletic/activity fees may still apply. Click here for details .
Applicable fees will be invoiced in July. Payment through MySchoolBucks is preferred.
Payment options include:
- Families are encouraged to set up an online payment account in MySchoolBucks to pay by credit card. Click here for directions on setting up your account.
- OR payment will be accepted at the school office by cash or check
- Please make checks payable to Lisle Community Unit School District 202
